While preventive exams are generally covered in full, most other visits will result in a partial payment from your health plan and a partial payment from you. We'll collect a credit card to hold on file for your account. Once your insurance has paid, you will receive a notification by email that the payment will be collected in 5 days. You'll receive another notification when the payment occurs and will also receive a receipt by email as well.
If we are out-of-network, OON, with your insurance plan, we'll be happy to supply a receipt so that you can apply for reimbursement. This is called out-of-network billing, and generally is reimbursed at a different rate. Some plans don't cover preventive services with an OON provider. Please call your plan or go online to understand your OON benefits before your visit. Here are some questions to ask.

The services provided as part of a well-child or preventive adult checkup are defined by the Centers for Medicaid Services and health plans. These checkups include an assessment of growth and development, screening for medical and psychosocial problems, anticipatory guidance to aid parents in the next stage of their child’s development, a review of vaccination history and status and administration of any vaccines necessary. We are happy to address questions that come up during the visit, and most can be answered simply and fall within the scope of this well child checkup or adult physical. Our patient portal is a convenient way to ask questions or seek clarification on your treatment plan after your visit. If you participate in our monthly Non-Covered Services program, it's also a simple way to ask for support from your medical team between visits when your question doesn't require a visit. Occasionally, health plans cover simple medical visits by portal as well. If your concern involves diagnosis and treatment, we will bill your insurance for a visit.
A One Sky Family Medicine physician is available to take urgent after-hours calls. These calls are generally to help you decide whether additional care is needed right away or if you can wait until the next available visit to come to the clinic. To reach the on-call doctor, please call our phone and select the option for urgent after hours calls.
Please be thoughtful with your calls and do not page for non-urgent issues, medication refills, lab orders or to schedule a visit. There is a $45 fee for after-hours pages. This service is not covered by health insurance plans.
One Sky Family Medicine provides a unique clinical experience and we value the depth of the relationships that we build with the people we serve. To that end, we generally spend significantly more time during each patient visit than is standard in health care today.
We require at least 24 hours' notice for cancellations, so that we may offer that appointment to another patient in our community. The 24-hour notice excludes weekend and holiday hours. If you need to cancel an appointment for Monday, please call before your appointment time on Friday.
The charge for a missed appointment or late cancellation is $60 for 30-minute visits and $100 for hour long visits, visits scheduled on a weekend, before 9 am, after 5 pm or for new patient visits. This fee will not be covered by your health plan.
